IMPORTANT HEALTH COVERAGE TAX DOCUMENTS

Van Buren County provides access to important tax documents related to your health coverage. If you were enrolled in a Van Buren County health plan, you may request a copy of the following tax forms:

  • Form 1095-B – For individuals with a MERS Health Care Savings Plan
  • Form 1095-C – For individuals who were eligible for or enrolled in regular health benefits through Van Buren County

Requests for Form 1095-B or 1095-C will be processed within 30 days of receipt. Statements may be provided electronically if requested.
